The Quick Case Creation form (CDAQCAS) is custom-designed to include only the fields your jurisdiction needs to enter cases quickly. The definitions below include all possible fields listed in alphabetical order. Note that the person responsible for setting up your system may have assigned different names to some or all of these fields.

If a list is available for a field or another form can be called to perform a search, the List Values option is enabled in the navigation frame.

If you enter a person ID that already exists in the system with some person information entered for it, that information automatically displays here. In addition, you will not be able to enter other person information here. When you try to do so, you receive a message in the hint text that the field is protected against update even though nothing is displayed in the field. When this happens, you can go to the PersonOrganization Maintenance form (CPAIDEN) to enter the information. Once saved, it displays on other forms in the system, including this one.

 

Atty Address Type

Code for the attorney's address type. List available.

If there is more than one address, the first active address displays. You can choose another or enter a new one of a different type.

Atty Area Code

Area code for the attorney's phone number. Maximum: 3 digits.

Atty City

City information for the attorney's address. If there is an address associated with the ID, the city is system-filled. Maximum: 20 characters. List available.

Atty Conf Ind

Check to indicate that the attorney's address is confidential. Users who are not set up on COACONF to view confidential addresses will not be able to see the address information.

If you have selected or entered a party and address type already in the system, the address may already be marked confidential. If you deselect the confidential check box, the address will no longer be confidential anywhere in the system.

Atty County

County of the address entered for the attorney.

Atty Email

Enter an email address for the attorney.

Atty ID

Enter an existing ID for the attorney. Search

Atty Party Type

Code for the party type of the attorney. List available.

The security level defined for the party type on the Party Type Code Rules form (CTRPTYP) is automatically associated with the attorney party type although it doesn't display on the form.

Atty Phone

Attorney's phone number. No dashes should be entered with the 7-digit number.

Phone types are associated with address types in the system. If you enter an ID with an existing address and a phone number is associated with it, it will be automatically entered.

Atty State/Prov

Attorney's state or province. If there is an address associated with the ID, the state or province is system-filled. List available.

Atty Street Line 1

Attorney's street address. If there is an address associated with the ID, the street address is system-filled. Maximum: 30 characters.

Atty Street Line 2, Atty Street Line 3

Additional lines for the street address. Maximum: 30 characters each line.

Atty ZIP/PC

Attorney's ZIP or postal code. List available. More about zip code entry

Calendar Type

Calendar type for an event. This allows added events to appear on calendar reports.

Case Description

Free-form text describing the case. Maximum: 50 characters.

Case ID

Enter an ID or NEXT to have the system generate one for you. Maximum: 15 characters.

Select List Values in the navigation frame to open the Case Query form (CDICASE) to verify a case ID you are entering does not already exist.

Case Type Code

Code for the type of case. List available.

Case Security

Number indicating security level of the case. 1 = lowest security (available to the most users); 9 = highest security (available to the fewest users).

Case Weight

Number indicating complexity of the case. 1 = least complex; 9 = most complex.

Claim/Value

Used in civil cases to indicate the amount of the claim in the case being filed.

Debit Account

Debit account from which fee payments are to be made. List available.

Event Code

Code for the event being scheduled. List available.

Event Text

Free-form field of up to 4000 characters for any notes that should be attached to the event.

Filing Date

Date the case is filed. Format: DD-MON-YYYY. The system defaults to the current date.

Filing Time

Time the case is filed. Format: HH:MM:SS. The system defaults to the current time.

Judge ID

Enter an existing ID for the judge. Search

Judge Party

Code for the party type of the judge assigned to hear the event being scheduled. List available.

The security level defined for the party type on the Party Type Code Rules form (CTRPTYP) is automatically associated with the attorney party type although it doesn't display on the form.

Person Checkbox

Select to indicate the plaintiff is a person; deselect to indicate the plaintiff is an organization. If an organization, enter the organization's name in the Plt Last Name field.

Plt Address Type

Code for the plaintiff's address type. List available.

If there is more than one address, the first active address displays. You can choose another or enter a new one of a different type.

Plt Area Code

Area code for the plaintiff's phone number.

Plt City

City information for the plaintiff's address. Maximum: 20 characters. List available.

Plt Last Name

Last name of the plaintiff. If an ID is entered which is already in the system, the last name is system-filled. Maximum: 60 characters.

Plt Middle Name

Middle name of the plaintiff. If an ID is entered which is already in the system, the middle name is system-filled. Maximum: 15 characters.

Plt Party Status

Status of the party on this case. List available.

Plt Party Type

Code for the party type of the plaintiff. List available.

Plt Phone

Prefix preferred by the plaintiff (if the plaintiff is a person).

Plt Street Line 1

Plaintiff's street address. If you enter an ID which is already in the system, any existing street address is system-filled. Maximum: 30 characters.

Plt Street Line 2; Plt Street Line 3

Additional lines for the street address. Maximum: 30 characters each line.

Plt SSN/Tax ID

Plaintiff's social security number or tax identification number. Maximum: 9 digits, no dashes.

Plt State/Prov

Plaintiff's state or province. List available.

Plt Status Date

Date the party status became effective for the plaintiff. Cannot be a date in the future or a date earlier than the case filing date. Format: DD-MON-YYYY.

Plt Status Time

Time the party status became effective for the plaintiff. Format: HH:MM:SS.

Plt Suffix

Suffix preferred by the plaintiff (if the plaintiff is a person)

Plt ZIP/PC

Plaintiff's ZIP or postal code. List available. More about zip code entry

Receipt Comment

Free-form text that will appear on the receipt for a debit account payment. If you enter a receipt comment, you must enter a debit account and there must be dockets with fees associated with the case.

Room Code

Room where the event being scheduled will be heard. List available.

Scheduled Date

The date the event being scheduled will be heard. Cannot be earlier than the case filing date. Format: DD-MON-YYYY.

Start Time

The starting time of the event being scheduled. Format: HH:MI:SS.

Trial Type Code

Code for the type of trial requested for the case. List available.
